The league doesn't need the Consent to Treat or Code of Conduct forms, but your team must be able to produce them if an emergency happens.
Birth Certificates?
No. Rosters are done online and USA Hockey keeps this information from year to year. If a player has no registration on file, the league will request a birth certificate for the player's file.
Submitting to the League
Send the official USA Hockey roster and the league's roster template, filled in, to Commissioner Ralph Bammert. Ask the Commissioner for the current template.
Manager Requirements
Background Check
A current background screening is required. Screenings are good for 2 years. Check your status in your portal on the USA Hockey website.
Background screening ↗SafeSport
Required every year. After the initial course, the next 2 years are a “refresher”; then the full course must be repeated.
